As a society for the prevention of cruelty to animals, the Martin County Humane Society (MCHS) has assumed responsibility for the welfare of every animal that enters our shelter. Our purpose is to place each animal in a responsible home where it will receive veterinary care when needed and where it will be controlled so as not to become a free-roaming animal or add to the surplus of animals already in the area. To ensure these ends, we have formulated this application to assist our MCHS members in the careful selection of home placements for our adoptable pets. Only those persons who are adults (18 or older) and who obtain satisfactory application approval from MCHS members will be eligible to adopt a pet from this shelter. Additional information other than this application may be required on an individual basis.
MCHS members reserve the right to refuse any adoption.

Is your home owned or rented?

Where will this pet be spending most of its time?

Where will this pet be sleeping?

Does your home have a fenced in yard?

In the unfortunate circumstances that you are unable to keep your pet or unable to care for your pet any longer, we prefer that you bring him/her back to our Humane Society. Should you need to give up your pet (regardless of how long you have had him/her, are you willing and able to bring him/her back to this shelter?
